You may remember that right before the flu season we sent out a survey to you about the flu shot. Thanks for those who answered. CCDC did some outreach about the flu and importance of getting the flu shots after we did that survey. Now we want to get just a little bit more information. We appreciate you taking just a few minutes to do one more very short survey about your 2011 experience with the flu an flu shots.
